When I was working with a very large company in the late 1990s, the press release schedule suffered from double-entry accounting—people on two continents were touching it and making changes, believing they were the ultimate authority on the document. As a result, version control was forked. We found out about the problem when a very, very important product was announced prematurely. Ooooops. Kind of hard to un-ring the bell, I guess.
